Phase I
Occurs during hospitalization and involves education regarding heart disease, tests/procedures, lifestyle modification and home instructions. The exercise portion of Phase I often involves low level exertion such as walking in the hallway. Most cardiac patients will benefit by participating in outpatient cardiac rehab after discharge from the hospital.
Phase II
Usually begins one to three weeks after hospitalization. Phase II classes are held three times per week for up to 12 weeks. The multi-disciplinary team works with the patient and their family to achieve each patient’s optimal level of health and fitness. All sessions are monitored and medically supervised. Education is provided in both group and individual format to help patients and families live a heart healthy lifestyle by taking control of factors that increase the risk of heart disease.
Phase III
Follows graduation from a Phase II program and is designed to move the patient toward independence in exercise and lifestyle management. The program is structured similar to the Phase II program with medical supervision and periodic rather than daily heart rhythm monitoring. Length of the program depends on several factors and can last indefinitely.

Will insurance cover the cost of the program?
Based on your diagnosis, Medicare and most insurance plans cover Phase II cardiac rehab. Phase III is not covered by Medicare and most insurance plans.
