Healthcare Failure Mode and Effects Analysis (HFMEA)

HFMEA

St. Edward Mercy Medial Center has an ongoing, proactive program for identifying and reducing unanticipated adverse events and safety risks to patients, visitors, physicians, co-workers, facilities and equipment. This is done through a process called Healthcare Failure Mode and Effects Analysis (HFMEA). The objective is to identify where a process can fail (Failure Modes) before the failure occurs. Each failure mode has a potential risk, with some risks more likely to occur than others; thus, the risk must be evaluated. HFMEA is performed using a multidisciplinary team approach with priorities based on potential risk factors. Variables (actions) are then placed into the process, in such a way that significantly reduces the likelihood of the failure happening in the future. The hospital identifies and conducts at least one HFMEA per year.
